Depends, but with alot of big charities out of every dollar often less than a cent goes to the actual cause. Some of them have boards of directors who are on salaries hundreds of thousands and in some cases millions of dollars.

For some, the percentage of your donation that actually goes directly to the cause is less than 40%, for others it is higher than 90% (like for the RNLI, an amazing charity!). After working for a few different charities, I always check this before donating!

A bit, not much. The only charities I know that are efficient about using their donations as intended rather for admin & advertising, are Oxfam and Corso (neither operating in the USA)
