Sometimes the "not okay" people are the nicest as they need "niceness" back more than anyone, I've found.

That's why it's so hard to know when those people might need a kind word.

Very true. Some people are very well practised at seeming ok. Indeed, some depressed people can seem the most content just before they attempt suicide.
