I always fed the leads in through the tip... if you press the eraser down and hold it, it should release and let you push the new lead in.

Take a needle poke it into the eraser at a 45* angle and pry it out. Put in lead and new eraser.

dig the eraser out with tweezers maybe a small screwdriver

Have you tried sticking a pin into what's left of the rubber and prising it out that way.

Hold down the rubber end, and carefully feed it into tip.
