Spanish flu, my uncle was a boy then and said nearly every other house had someone die. His mother went around caring for people who were sick helping with whatever she could And checking up on them. Walking around old cemeteries looking at gravestones and reading them made me realize how many died that year

Spanish Flu was way worse. They just did not have the media back then.

I'd say Bubonic Plague, but we know how to treat that now so it's a non issue for most countries.

Bubonic Plague is still very deadly if you get it, but in terms of pandemic and spread I think the Spanish flu.
