Custom made shirts are not difficult to find. But it really requires a visit to a tailor who will measure you. The on-line services are just not the equal.
Because I've for a long time liked the white collar and cuffs with a different body, I've soght out tailors for that reason among others, including the desired fit and details.
I used to see a tailor on my annual visit to New York City for that purpose.
I am not aware of custom sweater makers.

@DavidT8899 Any tailor will make up clothes for you, but you want a designer with pattern making experience and tailoring and/or dressmaking experience to create an original pattern and sew it. I was lucky with my dress. It was a simple design.
There are also weavers and people who will do custom knitting. Etsy might be the place to go for that.

I used to design my own patterns at times, and before I learned to sew them myself, my mother allowed me to take one of my designs to a tailor and we had it made up with my own fabric, which my father had brought back from India.
