No, but your question reminds me of a strange incident in my First Year of my upper school, so when I was 11. (In 1963)
We were all told to list things like our interests, family information, etc. in our lives outside school. It may even have used a form - I don't remember that, though. We were told it was part of an international pen-pal scheme, but I never heard anything more about it, and I don't think anyone else did either. I think it was really an internal routine for some school purpose I know not, perhaps a way for the teachers to know us better as people. Or even a discreet way for them to be aware of possible domestic problems that might affect us.

Yeah. A fair few. Lots over the years. My 1st one when i was 11 and she 12...we are still in touch now after 50+ years. X
