They talk to other people, read the news, and go to the doctors too. If you live in a city, it was impossible to not know.

I suppose those working for homeless charities tried spreading the word initially.
Whether or not it prevented any illnesses or deaths i don't know.

One of them heard the news,then started spreading it around the homeless community.
They may’ve realized it first, when many were hospitalized during the first breakouts. Several were quarantined in hotels.
