There are so many heroes and villains I'm sure one or two were just plain terrible xD

The Joker.
I get it, he "accidentally" fell into a vat of acid, but why didn't he die? Especially if you are going with the depiction in [i]Batman: The Killing Joke[/i]. By the time the acid filled up the helmet he would have run out of oxygen from inhaling the fumes, passed out, and drowned.
